THE ANNUAL WELLNESS VISIT

The health care reform law – the Patient Protection and Affordable Care Act – was passed in 2010 and requires insurance plans to pay for preventive services like shots and screening tests. This means that you and your loved ones may be able to get many important services at no cost to you. The Affordable Care Act also authorized an Annual Wellness Visit for all Medicare beneficiaries.

The Annual Wellness Visit is a comprehensive assessment questionaire that you must complete yearly. Completing this questionaire will help you to better understand about chronic diseases, your urgent health needs, learn better health behavior that will improve your health. Sharing this information will help us to provide you with immediate feedback in the form of education, counseling, or referral. After completing the annual wellness visit you can use our prevention plan to develop and improve your health status to delay the onset of disease.

Completing the questionaire assist you and your family to develop a “Personal Risk Plan” that will help keep you risk free. The assessment includes questions about predetermined risk areas and should take no more than 20 minutes to complete. ​ Each topic area is “actionable”, in that if the responses identify health or injury risks, you will be provided with reading materials with information and action to take to improve your health.
